You can either fill them out on your computer or print them to complete by hand.  If you fill them out on your computer, please return them to me as e-mail attachments or print them and bring them with you to your appointment. 

 

If you cannot open or print the forms please notify me in advance and they will be available to fill out when you come for your visit.
